Batch Request Entry System (BREQS)
About the service: The Batch Request Entry System (BREQS) is an off-line method of encoding requests for civil registry documents not only for those who are registered in the LGU but also those that may have been registered in other localities and submission thereof to an on-line Serbilis Outlet of the NSO. Copies of birth, death, marriage, annotated or endorsed documents, provided that said documents have already been issued by NSO previously, and the Certificate of No Record of Marriage (CENOMAR or "singleness), are the documents that can be applied through BREQS;
Fees:
" Fee for Requesting Birth, Death and Marriage in Security Paper - PHP 125.00 service fee plus P 140.00 for the document requested
" Fee for Requesting Certificate of No Record of Marriage (CENOMAR) - P120.00 service fee plus P 195.00 for the document requested
Registration of Foundling
About the service: Foundling is a deserted or abandoned infant or a child found, with parents, guardian, or relatives being unknown, or a child committed in an orphanage or charitable or similar institution with unknown facts of birth and parentage.
Reglementary Period and Place of Registration
Registration of the foundling in the Office of the Civil Registrar of the municipality shall be made by the finder/charitable institution within thirty (30) days from the date of finding/commitment of the child.
If the registration is made beyond the reglementary period, the concerned party shall be required to state in sworn statement the circumstances that caused the late reporting to the civil registrar.
Persons/Agencies Responsible to Report the Foundling
Immediately after finding a foundling, the finder must report the case to the barangay captain of the place where the foundling was found, or to the Philippine National Police whichever is neared or convenient to the finder. The finder must, then, commit the child to the care of the Department of Social Welfare and Development (DSWD), or to a duly licensed orphanage, charitable or similar institution. Upon commitment, the finder must give to the DSWD or the charitable institution his/her copy of the Certificate of Foundling, if the foundling was previously registered with the Office of the Civil Registrar.
In case the finder is awarded the custody of the foundling by the proper authority, he/she shall give a name for the child and shall report the same to the civil registrar of the city or municipality where the child was found. Otherwise, the giving of name to the child and its registration as foundling shall be the responsibility of the Department of Social Welfare and Development or of the orphanage or charitable or similar institution where the child was committed.
Requirements: (1) Affidavit of the finder (2) Certification of the barangay captain or police authority regarding the report made by the finder, stating among other things, that no one has claimed the child or no one has reported a missing child whose description may be the same as the foundling as of the date of certification.
