Securing Mayor's Clearance and Certificate of Good Moral Character
About the service: Individuals need to secure a Mayor's Clearance before they can apply for firearms license, employment and for those who are applying to the Philippine National Police (PNP).Some schools, local and overseas employment agencies require their students/ recruits to secure a Certificate of Good Moral Character before they are allowed to enroll or apply.
Requirements: (1) Original copy of Police Clearance (2) Original copy of court clearance (for clients securing firearms license) (3) Official Receipt from the Municipal Treasurer's Office
Fees: P 100.00
Securing Permit on all advertising and promotional materials
About the service: Corporations, companies, groups and other entities wishing to promote or advertise their product(s) or service(s) to the public through the installation/posting of billboards, signages, streamers, posters and/or fliers and other similar materials need to secure a permit. The Municipal Government removes advertising materials if they are installed or posted without the necessary permit.
Requirements:
For Streamers, Posters and Flyers: (1) Letter Request addressed to the Municipal Mayor stating the total number of streamers, posters or flyers and the period/duration of posting. (2) Official Receipt from the Municipal Treasurer's Office
For Billboards and Signages: (1) Letter Request addressed to the Municipal Mayor copy furnished the Municipal Engineering and Planning Office (2) Design, details and specifications, and the sketch plan of the site where the billboard will be installed (3) Document showing consent of the property owner, if billboard will be installed on private property (4) Affidavit of undertaking to assume all obligations and liabilities caused to any third party by reason of such project.
Fees:
Streamers - Php 400.00 Succeeding piece - Php 100.00
Erection of supports of any signboard, billboard marquee and the like:
a. up to 4 sq. m of signboard area - Php 300.00
b. every sq. m. or fraction thereof in excess of 4 sq. m. - Php 100.00
Installation Permit Fee, per sq. m. of display surface or fraction thereof:
Business signs:
a. neon - Php 30.00
b. illuminated - Php 20.00
c. others - Php 12.00
d. painted on - Php 8.00
Securing permit to conduct motorcade or parade
About the service: All groups and entities that wish to stage a motorcade/ parade and/or procession around the municipality are required to get a permit from the Office of the Mayor prior to the scheduled activity. This ensures coordination and orderly traffic management during the activity.
Requirements:
For Private Entities: (1) Letter of Request addressed to the Municipal mayor indicating the schedule of motorcade or parade, planned route and purpose of the activity. (2)Official Receipt from the Municipal Treasurer's Office
For Government/ Religious entities: (1) Letter of Request addressed to the Municipal mayor indicating the schedule of motorcade/parade/ procession, planned route and purpose of the activity.
Fees: For private entity - P 100.00 / For government or religious entities - no fee
Public Customer Assistance
About the service: Residence of the municipality of Daet and other clients may request the Municipal Mayor for referrals, recommendations, endorsements or communications for any of the following: (1) Medical/Burial Assistance (2) Job Recommendations (3) Endorsement for transfer (4) Transportation Discount
Requirements:
For Medical/ Burial Assistance: (1) Medical certificate/ prescription (2) Voter's Certificate (3) Barangay Certificate of Indigency (4) Death Certificate (authenticated/certified xerox copy) - for burial
Job Recommendations: (1) Complete Bio-data / Resume / Curriculum Vitae (2) Police Clearance (3) Past Employment records (if any) (4) Other relevant documents
Endorsement for transfer: (1) Letter of request for transfer stating therein the reasons for the request (2) Certified Service Record (if available)
Transportation Discount: (1) Community Tax Certificate (2) Voter's identification
Other Inter-Agencies Recommendations: (1) Letter of request stating the reasons for the request (2) Other relevant documents
Using Government Facilities
About the service: The Municipal Government of Daet offers the use of the following facilities for seminars, conferences, gatherings and other activities such as (1) Daet Elevated Town Plaza (2) Daet Kalinangan Center (3) Sound System (4) Service Vehicle
Requirements: (1) Letter request addressed to the Municipal Mayor, specifying the government facility/ ies to be used date, time and purpose. (2) Clients may go directly to the Daet Kalinangan Center in Barangay Calasgasan to avail its facilities.
Fees: Daet Kalinangan Center // Room Rate - Php 75.00/day; Conference Hall - Php 1,000.00/day
