Application for Marriage License
About the service: Marriage is a special contract of permanent union between a man and a woman entered into in accordance with law for the establishment of conjugal and family life. It is the foundation of the family and an inviolable social institution whose nature, consequences, and incidents are governed by law and not subject to stipulation, except that marriage settlements may fix the property relations during the marriage within the limits. In application for a marriage license, each of the contracting parties shall file separately a sworn application with the Municipal Civil Registrar of the place where either or both of the contracting parties reside. Marriage license shall be valid in any part of the Philippines for a period of one hundred twenty (120) days from the date of issue, and shall be deemed automatically cancelled at the expiration of the said period if the contracting parties have not made us of it.

Registration of Birth and Marriage Certificates
About the service: The birth of the child shall be registered within thirty (30) days from the time of birth at the Office of the Civil Registrar of the municipality/city where the birth occurred. In ordinary marriage, the time for submission of the Certificate of Marriage is within fifteen (15) days following the solemnization of marriage while in marriage exempt from license requirement, the prescribed period is thirty (30) days, at the place where the marriage was solemnized.

Registration of Death Certificate
About the service: Death is a permanent disappearance of all evidence of life at any time after live birth has taken place. It shall be the responsibility of the physician last attended the deceased or the administrator of the hospital or clinic where the person died to prepare the proper death certificate and certify as to the cause of death. The death certificate shall then be forwarded within forty eight (48) hours after death, to the health officer who shall examine the Certificate of Death and then affix his signature in the appropriate box and shall order its registration in the Office of the Civil Registrar.
It shall be the responsibility of the nearest relative or person who has knowledge of the death to report the same within forty eight (48) hours if the deceased died without medical attendance. The health officer shall examine the deceased and shall certify as to the cause of death and direct the registration of the death certificate to the Office of the Civil Registrar within the reglementary period of thirty (30) days.

Requesting a Certified Copy of Birth, Marriage and Death
About the service: Civil registry documents such as birth, marriage and death certificates may be availed of by securing a certified transcript or photocopy from the office of the Municipal Civil Registrar.

Registration of Court Orders / Decrees
About the service: Like other civil registry documents, court decrees concerning the status of a person must be registered in the civil registrar's office where the court is functioning.
The following are registrable court decrees:
" Decree of Legal Separation
" Declaration of Nullity of Marriage
" Court decision/order to correct or change entries in any certificate of birth, marriage or death
" Declaration of presumptive death
" Repatriation or voluntary renunciation of citizenship
" Court decision recognizing or acknowledging natural children or impugning or denying such recognition or acknowledgment
" Judicial determination or maternity affiliation
